What GLP-1s actually cost in 2026

The same brand-name drug appears in this research at $149 a month and at $1,599 a month. Here is what explains the difference, and how to work out your real number.

The four prices for the same drug

There is no single price for a GLP-1. There are four, and which one you get depends on the route rather than on the medication.

Work out your real number

Start from the maintenance dose, not the starting dose. Lilly’s ladder runs $299 → $399 → $449 as you titrate.
Add every mandatory recurring fee. Four services in this research charge one on top of the medication.
Check whether the headline is introductory. NovoCare’s $199 pen price is first-two-months, new-patients-only; WeightWatchers is $25 for two months then $74; Sesame’s $199 becomes $349.
Check the billing cycle. Brello bills $499 every 11 weeks, which is four charges a year rather than three.
Multiply by twelve. That is the number that matters, and it is rarely the one on the landing page.

Annual figures assume a stable dose for twelve months, which is optimistic — most people titrate upward, and several of these prices carry stated expiry dates.

What compounded means, and where the rules are heading

Compounded GLP-1s are not FDA-approved products. Several of the services selling them say so on their own sales pages: Alloy states that “unlike FDA-approved drugs, compounded medications are not reviewed by the FDA for safety, effectiveness, or quality,” and Noom draws the sharper distinction — its product is “produced in an FDA-registered facility but has not been reviewed by the FDA for safety, efficacy, or quality.” A registered facility is not a reviewed medicine.

This tier is now under active enforcement. FDA announced 30 warning letters to telehealth companies in March 2026 and a further 25 in June, over false or misleading marketing of compounded GLP-1 products — including claiming or implying FDA approval, and describing a compounding pharmacy or outsourcing facility as “FDA-approved” or “FDA-licensed,” which is an authorisation the agency does not grant. Five providers reviewed on this site are named recipients; their reviews say so at the top.

The larger development is structural, and it is worth quoting exactly. In a notice published on 1 May 2026 (91 FR 23431, Docket FDA-2018-N-3240), FDA said it “proposes not to include on the 503B Bulks List: semaglutide, tirzepatide, and liraglutide.” That list is the set of substances for which FDA has found “a clinical need for outsourcing facilities to use in compounding”. Leaving all three off it would remove the legal basis for 503B outsourcing facilities to compound them in bulk — which is most of the cheap tier on this site.

It is a proposal, not a rule, and it is not settled. FDA extended the comment period on 26 June 2026, moving the deadline to 30 July. Nothing has been finalised as of this update. But anyone signing a twelve-month prepaid compounded plan should understand that the supply may not be lawful for the length of the plan, and should ask the provider what happens to the balance if it is not.

Because one is the manufacturer’s self-pay programme and the other is a retail cash price with no programme applied. Wegovy appears in this research at $149/month through NovoCare and $1,599/month through Lemonaid. Always check whether a service can route you to the manufacturer pharmacy.

What is the cheapest a GLP-1 gets?

With commercial insurance that covers it, as little as $25/month via the manufacturer savings cards. Self-paying, $149/month for the Wegovy pill. Compounded, about $139/month all-in at Mochi Health.

Do prices rise as I increase the dose?

Usually. Lilly publishes $299 at 2.5 mg rising to $449 at 7.5 mg and above; Sesame’s Wegovy pill goes from $149 to $299 for higher doses.
